How to Use Patient Surveys Step 1: Create a Survey Record You can create survey records whenever you want to collect patient feedback. Multiple survey records can be added for a single appointment — for example, right after the consultation and again after follow-ups. There are three ways to add a survey record: From Appointments Page : Click on the appointment → choose ‘Add Survey’ from the pop-up menu.
